When calling for an appointment, please
be prepared to provide the following:
•Patient’s name
•Social Security number
•Address
•Phone number
•Date of birth
•Type of insurance
•Reason for appointment
•Referring physician’s name
What to Bring to Your Appointment
•Your insurance card
•Referral forms, if required by your insurance
provider
•The dose and frequency of any prescription and/or
over-the-counter medications you are taking
•Previous urologic evaluations or tests
•Relevant medical records our office may not have
Check-out
After your appointment, please stop by the check-out
desk to schedule follow-up appointments or to make financial
arrangements for any remaining self-pay balances.
