Appointments

Appointments can be made during office hours by calling 316.636.6100. Our office hours are Monday through Friday, 8 a.m. to 5 p.m. When calling for an appointment, please be prepared to provide the following:

• Patient’s name
• Social Security number
• Address
• Phone number
• Date of birth
• Type of insurance
• Reason for appointment
• Referring physician’s name

What to Bring to Your Appointment:

• Your insurance card
• Referral forms, if required by your insurance provider
• The dose and frequency of any prescription and/or over-the-counter medications you are taking
• Previous urologic evaluations or tests
• Relevant medical records our office may not have

Check-out

After your appointment, please stop by the check-out desk to schedule follow-up appointments or to make financial arrangements for any remaining self-pay balances.

Completion of Forms by Clinic Staff

There will be a $10 fee collected in advance for all Disability, FMLA or other similar forms that are requested to be completed by the clinic staff. Please allow 5 business days for completion of these forms.

After Hours & Emergency Care

If you are experiencing an emergency, dial 911 or head to the nearest emergency room. For all other calls, an on-call physician is available 24-hours a day, 7-days a week at 316.636.6100.

Prescription Renewals

No narcotics prescriptions will be phoned in on the weekends between Friday at 5 p.m. and Monday at 8 a.m. Please make sure all your refill needs are taken care of during normal business hours.

Financial

All office visits are payable at the time of service. We accept cash, checks, and most major credit cards and debit cards. Please contact our billing office should you have any financial questions or concerns.

Patients without insurance are required to pay a minimum payment of $75 prior to seeing the doctor. Arrangement will be made at check-out immediately following your first visit to pay the remainder of your balance.
