Wichita Urology requires that all insurance co-pays are paid at the time of your office visit. You may pay by cash, check, VISA or Mastercard.

Patients without insurance are required to pay a minimum payment of $75 prior to seeing the doctor. Arrangement will be made at check-out immediately following your first visit to pay the remainder of your balance.

Completion of Forms by Clinic Staff

There will be a $10 fee collected in advance for all Disability, FMLA or other similar forms that are requested to be completed by the clinic staff. Please allow 5 business days for completion of these forms.

Narcotics Prescriptions & Refills on Weekends

No narcotics prescriptions will be phoned in on the weekends between Friday at 5 p.m. and Monday at 8 a.m. Please make sure all your refill needs are taken care of during normal business hours.
